This position is for a working supervisor for Revere Health's CCM program. The successful candidate will work with physician and director leadership to define and implement a population based CCM program. The candidate will supervise the CCM team of RN's and ensure the goals of the CCM program are met. The candidate will be required to set data driven CCM program goals, ensure the team is working towards those goals and report outcomes to director and physician leadership. The Candidate will also provide CCM visits to a panel of patient needing CCM outreach. Adaptability and flexibility is key in this position.

Essential Functions: 

  • The CCM Supervisor develops and manages the CCM program to improve the health of Revere Health's high risk patient population
  • Provides supervision of the CCM team of RN's and coordination of patient outreach and visits
  • Ensures patient-centric care plans are implemented, followed and patient resources are identified
  • Sets and manages goals of the CCM program to ensure success and improved population health outcomes
  • Works part time as a CCM nurse for a small patient panel
  • Ensures all CMS requirements for CCM visits are followed and that proper care is provided and coding is followed per guidelines
  • Works closely with physicians and ensures communication between CCM team and the physician is on-going and constructive
  • May perform additional duties as assigned within the scope of responsibility
  • This position will require travel to patient homes and various Revere Health Clinic location.

Minimum Qualifications: 

  • Registered Nurse with minimum of 10 years clinical care experience or five years as a CCM Supervisor is required 
  • Must have a valid Utah nursing license
  • Previous Care Management experience or training required
  • Knowledge of CCM processes and CMS requirement required
  • Supervisory experience required
  • Excellent verbal and written communications skills
  • Ability to work with physicians and other key departments to improve processes
  • Excellent time management skill and the ability to work with minimum supervision
  • Excellent analytical and spreadsheet skills
